Terms and Conditions for Text Message Communications

Level Up Mental Health Collaborative

1. Purpose:

Level Up Mental Health Collaborative ("Level Up MHC") uses text messaging to enhance client communication and support. This may include appointment reminders, check-ins, and brief, supportive messages.

2. Consent:

By providing your mobile phone number and consenting to receive text messages, you agree to receive these communications from Level Up MHC. You can consent verbally during your intake phone call. By providing consent, you are also consenting to any standard text message and/or data rates that may apply. No mobile opt-in data will be shared with third parties. 

3. Frequency:

The frequency of messages will vary based on your needs and preferences and will generally not exceed 1-3 messages per week. We will avoid sending excessive messages or contacting you outside of reasonable business hours.

4. Opting Out:

You can opt out of text messages at any time by:

• Contact your therapist or our office directly at 701-757-0477.
• Text STOP to opt-out

5. Data Privacy:

We value your privacy. Your mobile phone number will only be used for sending messages related to your care at Level Up MHC. We will not share your information without your consent - No mobile opt-in data will be shared with third parties.

These terms may be updated, and any changes will be communicated to you through your client portal or other appropriate channels.

Privacy Policy for Text Message Communications

1. Information We Collect

When you opt in to receive text messages, we collect your mobile phone number. We may also collect your name and any other information you provide when you contact us via text message.

2. How We Use Your Information

We use your information to:

• Send you appointment reminders, check-in messages, and occasional supportive messages.
• Respond to your inquiries and requests for information.

3. Disclosure of Your Information

We will not share your information with third parties without your consent, except as required by law or to comply with legal process. 

4. Data Security

We take reasonable measures to protect your information from unauthorized access, use, or disclosure. However, no method of transmission over the internet or method of electronic storage is completely secure. No mobile opt-in data will be shared with third parties.

5. HIPAA Compliance

Our text messaging practices are designed to comply with the Health Insurance Portability and Accountability Act (HIPAA) to ensure the confidentiality and security of your protected health information. 

6. Accessing and Updating Your Information

You can access and update your communication preferences, including opting out of text messages, through your HIPAA-secure client portal or by contacting our office.
